Charges
Charges are based on the type and cost of services
required, the time of the professionals who treat you, and the complexity of your medical problem. Our physicians’ appointments are scheduled so there will be ample time to discuss your medical care. Our physicians
may also spend significant time, as required for diagnosis and treatment, conferring with other specialists who have seen you, discussing findings with radiologists and pathologists who performed your tests,
and making reports for your medical record and your referring physician. Your total charge will reflect this work and time, and may combine charges for both physicians’ services and hospital services, such as
X-ray and laboratory work.
Insurance/Billing
Please bring your insurance card(s)/policy information with you to all appointments. If you belong to a managed care plan (HMO, PPO, etc.) you will
probably need to bring a referral form, pre-certification authorization, and/or an authorization number. Some managed care plans require a co-payment at the time of service.
Duke will file claims to your insurance company or companies on your behalf. We will also respond to any request from your insurer for additional
medical information. However, you are responsible to follow up on the claim and obtain payment from your insurance company if it does not pay promptly.
You will be asked to pay your deductible and coinsurance at the time of your visit. You are ultimately responsible for payment, regardless of
insurance coverage or claims handling.
