Eligibility

1. Members of the PLLC are eligible for full benefits on the effective date of their membership.
2. Associate Members (full-time Professional or Clinical employees) of the PLLC are eligible for full benefits on the effective date of employment.
3. Employees (full-time Senior Administrators - Duke University Level 98 (i.e. PDC Directors and PDC Departmental Business Managers) of the PLLC are eligible for full benefits on the effective date of employment.

 

Coverage

• Dental - Group Dental Expense Insurance Benefits - $50.00 deductible per calendar year for each family member; then 80% coverage of usual and customary charges.
• Disability - Group Term Disability Benefits - 90 day elimination period then 60% of basic monthly earnings is payable while totally disabled. The maximum benefit is $20,000 per month.
• Life and Accidental Death & Dismemberment (AD&D) –
