Items to Bring With You
Personal Social Security numbers for patient and spouse.
Retirement dates for patient and spouse (if applicable).
Medical Name and address of your referring physician or the name and address of the physician you would like to
receive the reports from your Duke visit. List of any medicines you have been taking at home, and/or the medicines themselves in their prescription containers.
List of
any known food or drug allergies Any pertinent medical information, records, or test results (such as X-rays, EKG tracings) given you by your personal physician.
For a child, in addition to the above: immunization records birth dates of child, parent(s) and/or guardian(s) Social Security numbers
for child, parent(s) and/or guardian(s).
Financial Your health insurance card and any required authorization, authorization information, pre-certification information or HMO
referral forms.
If your treatment is covered by Worker's Compensation, a letter of authorization From your employer.
Financial resources to cover a possible deposit as explained in
the financial section about insurance claims and billing (cash, personal check, Visa or Mastercard, and Discover are acceptable methods of payment).
You (and any
family members or friends who accompany you) may want to bring reading material or handiwork for any waits you encounter.
