Pre-registration

 You may be contacted by phone by our Pre-Registration Department in advance of  your appointment. Our hours of operation are from 8:00 a.m. to 5:00 p.m. The  representative will ask questions to update or complete your registration,  gather any insurance information needed, and help with any managed care  authorizations that may be required. Telephone registration will significantly  reduce your wait time in the clinic. When you arrive for your appointment, the  information is already on file and only a few forms must be signed.

Even  if you have pre-registered, please bring your insurance card(s) when you come in  for your scheduled appointment so we can verify that the insurance information  has been recorded correctly in our system.

Some of our patients may  receive a pre-registration form in the mail. If you do receive a form, please  complete it and return it to us prior to your visit. We will update our system  with the information you provide.

Registration

 If you have not pre-registered, a Patient Relations Representative will interview you to gather personal and insurance information and to estimate total charges for the visit. The registration process may take 30 to 45 minutes. We will issue you a Duke Medical Record Number to identify you throughout the Duke system.

We will also enter all your insurance information into our  records. In certain instances we may have to verify insurance coverage. We will need your signature on various forms. These steps are necessary to ensure you  get maximum benefit from your insurance.

Please plan to arrive at least 45 minutes prior to your scheduled appointment. After you have registered, you  will be directed to your physician's clinic area.
